Insurance: 

I am an in-network provider for Aetna, Anthem, Florida Blue, some Blue Cross and Blue Shield, Cigna, Oscar, Oxford, United Healthcare, all Optum affiliates, and VA Community Care.

 

Cancelation Policy: 

Cancellations must be made 24 hours before your scheduled appointment. If not, you will be charged $125 for the missed session. Payment must be made at the time of the appointment unless otherwise arranged. If you cancel several consecutive sessions, it is likely that I will not be able to guarantee you a spot due to the high demand for sessions.

Payments: 

In order to reserve an appointment with Mara Grace Counseling, a credit card on file is required. We accept many forms of payment such as Vcards, virtual insurance credit cards, credit cards, HSA, cash, and checks at the time of your scheduled appointment.  

I am an out-of-network provider for other healthcare plans, including Medicare and Medicaid. If you have a PPO, you can contact your insurance company to ask them what they cover for out-of-network providers. I can provide you with an invoice or superbill (an itemized statement) to obtain reimbursement from your insurance company if you have out-of-network benefits. All payments are due at the time of service unless otherwise arranged. I recommend contacting your insurance provider directly if you don't know if you have out-of-network benefits.
